By default, My designs and Order again live in the buyer's WooCommerce account, so saving there requires being logged in. A design left without checking out is still kept as a "Designs in cart" record even for a guest; letting a guest explicitly save a named design without logging in at all is custom development, not a default setting.

Can a buyer reorder a design that is months old?

Yes. A saved design stays in My designs until the buyer deletes it, and Order again puts it back in the cart with its original print options whenever they return to it.
